Southeast Community College is committed to a safe, welcoming environment for students, faculty, and staff. Use the forms below to report incidents or concerns. Detailed information is helpful, but you are not required to complete every field. Anonymous reports are accepted; however, they may limit the College's ability to investigate and respond.

This is not a 911 or emergency reporting site. If you witness an incident in progress or feel your safety is at risk, call 911 immediately.

Student Incidents & Concerns – All Locations
Academic Integrity Violation Use this form to report a violation of SCC's academic integrity standards, including:
  • Academic dishonesty or cheating
  • Plagiarism
  • Disruptive classroom behavior
  • Concerns about a final grade
Accommodations Concern Use this form to report an issue related to disability accommodations or accessibility, including:
  • A concern about how an accommodation was implemented
  • A request for review of an existing accommodation decision
  • A physical accessibility barrier in a campus facility
Student Care Report Use this form to report concerns about a student's well-being, safety, or behavior so they can be connected with appropriate support. Examples include:
  • A student showing signs of distress, anxiety, or crisis
  • Concerning behavior that may affect the student or others around them
  • A student who has gone missing or stopped attending without explanation
Student Conduct Incident Use this form to report a violation of SCC's Student Code of Conduct. For more information, refer to the Student Code of Conduct Violations webpage

Accident / Injury Report
Use this form to report any physical injury or accident that occurred on campus or at an SCC-sponsored event, including:
  • Personal injury (slip, fall, or other physical harm)
  • Vehicle accident on SCC property
  • Medical emergency or medical incident
  • Athletic or activity-related injury

Campus Safety & Security Report
Use this form to report safety concerns, criminal activity, or security-related incidents on SCC property or at SCC-sponsored events, including:

  • Damage to or theft of property
  • Suspicious persons or activity
  • Crimes that may be required to be reported under the Clery Act (such as assault, burglary, or harassment)

Use this form for concerns or suggestions that do not fit any other category on this page, including feedback about:

Human Resources – All Locations
Employee Accommodation Request Use this form to request a workplace accommodation related to a disability or medical condition.
Family Medical Leave Act (FMLA) Request Use this form to request leave under the Family Medical Leave Act for a qualifying health condition, the birth or adoption of a child, or to care for a family member.
Work Related Injury/Accident Use this form to report an injury or accident that occurred in the course of your work at SCC. This is separate from the campus Accident/Injury Report and is specifically for employee workers' compensation purposes.

 

 
Title IX Incidents & Concerns – All Locations
Title IX, ADA/504, and Civil Rights Discrimination Reporting Use this form to report discrimination or harassment based on a protected characteristic, including:
  • Sex-based discrimination
  • Sexual harassment
  • Sexual misconduct or sexual violence
  • Retaliation for reporting a concern
  • Disability-based discrimination (ADA/504)
  • Discrimination based on race, national origin, or other civil rights protections
